Health History

Before your first session you will be asked to fill out a Client Intake Form so please arrive 10-15 minutes before your appointment time. 
To provide proper services at all follow up  sessions, it is the your responsibility to always inform the me of any and all new and existing health conditions.

Confirmations

All confirmations will be sent to you 48 hours prior to your appointment. You will be able to choose how you receive your confirmation when you schedule your session. 

CancellationS

Cancellations are sometimes unavoidable. Please call me within 24 hours of your appointment to allow me ample time for filling that appointment as well as to avoid a fee.

Tardiness

Please arrive on time for your service. If you are late for your appointment, I will provide you as much time as we have available within your scheduled time slot. You will, however, still be charged the full amount for your scheduled session.

Payment is due upon receipt of services. Cash or credit cards are accepted.

Sexual Misconduct

It's unfortunate that I have to put this in, but I don't want there to be any possibility of a misunderstanding. Sexual misconduct is absolutely forbidden. It is important that all of my clients understand that any illicit or sexually aggressive remarks, advances or gestures will result in the immediate termination of the session and you will still be liable for full payment of the scheduled appointment.

Physician Referral

A physician referral is generally not required in order to receive massage. However,there is always a chance that after speaking with you and reviewing your health history information, a determination may be made prior to your session that a doctor’s note is required (i.e. cancer treatment, recent car accident, recent surgery, ect.). To avoid any issues please discuss any of these things with me when making your appointment.
